Third Street Clinic services are available to residents of Grand Forks county and Polk county who meet eligibility requirements with no restrictions based on race, religion, sex or age.

Applicants will need to bring identification, verification of all household income and of residency. Applicants must be a resident of Grand Forks county or Polk county for 30 days for all non-emergencies. Exceptions may be made on emergency basis only and with approval of the TSC medical director. A one time appointment will be granted.

Once established, applicants will be eligible for our program for six (6) months, after this time they will need to renew for eligibility.

1)     The Third Street Clinic staff will make an appointment for the client to see a physician at Altru Clinic, Altru Family Medicine Center, Grand Forks Family Medicine Residency, Valley Vision or individual dental offices.
2)     Clients will be assigned a primary care physician for their initial health care visit and follow-up care. Clients will be given a voucher to present at registration for each appointment.
3)     Clients will be assigned a primary dentist for emergency dental care. The dentist will refer the client to a specialist if necessary. Clients will be given a voucher to present at registration for each appointment.
4)     Third Street Clinic provides a voucher for medications listed on our formulary. Clients can have their prescription filled at any Grand Forks pharmacy. There is a $2.00 co-pay for each prescription. TSC will help with six prescriptions per condition only.
